What exactly did you do...?
That error is from the Manage HTPasswd File webmin module?
That probably be a problem of third-part module and not SMS's, I'll check it though in case a perl module is missing.
There is also under "other" Protected Web Directories module that do exactly the same, with more options.

[EDIT] I see what's the problem, when add new user.
Despite the error from htpasswd-file module the user is created, just output the error from Manage HTPasswd File webmin module.
You might as well delete Manage HTPasswd File webmin module, I know I 'll since there is a module that already doing that.

Correct- I created the user in the "User/Group" menu.
yes - I noticed that the user is created anyway.

I will try deleting the Manage HTPasswd File webmin module.
