I have uninstalled and reinstalled both mysql and mysql-data, but did not fix. Could this be a upgrade issue?
never mind, found another post of another user having same issue. Remove user.MYI , user.MYD, and tables_priv.MY* and reinstalled mysql_data fixed the issue
